REFERRAL FOR CONSULTATION

Your dentist will refer you and at your initial appointment, Dr Tarek Abdel - Naby will perform a thorough examination, explain his diagnosis and propose treatment that you may need. Some minor surgical procedures can be performed on the same day as the consultation.

For more extensive procedures and those which may require intravenous sedation, an initial consultation and examination is done and the surgical treatment is scheduled for a later date.

On the day of your consultation, please have the following information with you:

The referral request from your dentist

Any x-rays that your dentist gave you, additional x-rays may be required. The name and dosage of any medication you are taking (prescription and non-prescription)Dental insurance information.

Our office is open Mon-Fri from 8am to 5pm. We will make every attempt to schedule your appointment as soon as possible. Please advise us if you are experiencing discomfort or having an emergency that requires prompt attention, every attempt will be made to see you that day.

FINANCIAL SERVICES

In order to provide the finest care at the most reasonable costs payment is expected at the time services are rendered. You are responsible for all fees and charged regardless of your insurance coverage.
